Thank you for choosing Dr. Balyeat for your healthcare needs. The following will
outline our financial policies to assist you in understanding your rights and to
avoid any misunderstandings. If after reviewing our policies you still have
questions, please do not hesitate to ask for assistance.

Payment for services is the financial obligation of the patient or responsible
party.  We will file a claim with your insurance carrier as a courtesy but
understand that you are ultimately responsible for any charges incurred NOT
YOUR INSURANCE COMPANY. It is also very important that you provide our
office with current copies of all medical insurance cards pertaining to plans with
which you have coverage.



We are a participating provider with Medicare and will accept assignment for
Part B patients only.
You will be expected to pay your deductible, co-
insurance and any service that Medicare deems as “non-covered”
. In
addition to filing your claims with Medicare, we will file with your secondary
insurance if applicable.



We are a participating provider with Oklahoma Medicaid and we will file your
claim.  If you are enrolled with the HMO plan under Medicaid called SoonerCare,
it is your responsibility to present your referral to our office.
You will be
responsible for any applicable co-pays and co-insurance
.



Dr. Balyeat is a participating specialist with most managed care plans. He
accepts Community Care, Pacificare/Secure Horizons, United Healthcare,
Aetna and many others. Please verify with the office staff that he accepts your
particular plan BEFORE receiving treatment to avoid any misunderstandings. If
Dr. Balyeat participates in your plan, we will file your claim and accept
assignment according to the managed care agreements. You will be
responsible for any applicable co-pays, coinsurance or services that are
deemed as “non-covered” by your plan.  
Payment of office examination co-
pays are expected upon check-in to the waiting room.



In the event that you do not have insurance coverage, payment is expected at
time of service unless prior arrangements have been made. If you feel you do
not have the ability to pay, please discuss this with any staff member prior to
your visit.  We may be able to direct you in obtaining financial assistance. We
will provide you with a monthly statement showing your outstanding balance and
will provide detailed billing upon request.



We accept Cash, Checks, Debit Cards, Visa, Mastercard, Discover and
American Express.
